29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) At Magic Tilt Trailers, Inc., located at 2161 Lions Club Road, Clearwater, FL, at or near the Service Shop and Shipping building: On or about April 17, 2024, the employer exposed employees to struck-by and crush hazards in that the fork attachment on a Komatsu forklift had been modified to included removal of one fork and the attachment of a ball hitch to the end of the other fork.
