29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) On or about April 11, 2024, and at times prior, the employer exposed employees to roll-over, struck-by, and crushing hazards by placing metal bolts inside the tire treads on the CAT Forklift to ensure the balding tires still maintained grip and traction without the written approval of the manufacturer. (b) On or about April 11, 2024, and at times prior, the employer exposed employees to roll-over, struck-by, and crushing hazards by utilizing fork extensions on the CAT Forklift that affected capacity without the written approval of the manufacturer.
