29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: At a worksite located at or near 39.185939, -93.868953 in Lexington, MO: Employees were exposed to both physical and health hazards in the performance of their job duties. The employer failed to instruct employees on the condition(s) and the regulation(s) applicable to their work environment to control or eliminate any hazard(s) or other exposure to illness or injury. This was last demonstrated on or about March 11, 2024, when employees were working to replace a sanitary sewer pipe inside a vault and employees had not been trained on all the potential hazards that were present or could be present during their work. This included, but was not limited to electrical, noise, toxic vapors or gases, chemical, and biological agents.
