29 CFR 1910.28(b)(3)(iv): The employer must ensure that each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used. On or about March 12, 2024, with respect to an establishment located at 1001 Discovery Road, Green Bay, WI 54311, employees working from a maintenance platform on the backside of Rewinder 2 were not protected from falls when the opening for a fixed ladder was not equipped with a self-closing gate or offset to prevent falls through the opening.
