1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a) On or about February 2, 2024, at the worksite 744 S Meadow St, Ithaca, NY 14850: Employees were engaged in roofing activities on a flat roof, 11.5 feet above the ground. The employer failed to protect employees from falling by use of guardrail systems, safety net systems, or personal fall arrest systems. Hale Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 1593307, citation number 2, item number 1, and was affirmed as a final order on August 9, 2022, with respect to a workplace located at 104 Lake Avenue, Ithaca, NY 14850.
