Custom Valve Repair, LLC 3589 Harlansburg Road New Castle, PA 16101, on or about 01/25/2024, and times prior thereto: The emergency stop button on the Reading 5-ton hoist serial #13304 pendant was missing. Hoist pendant was used to lift equipment including but not limited to, large control valves. b.) Custom Valve Repair, LLC 3589 Harlansburg Road New Castle, PA 16101, on or about 01/25/2024, and times prior thereto: Periodic inspections are not performed or documented on the hoists. Hoists include, but is not limited to, Reading 5-ton hoist (Serial number 13304), and Reading 5-ton hoist (Serial number 13305).
