29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): Location: 195 North Main Street, Boscawen, NH - Back Warehouse On or about January 4, 2024, employees were exposed to struck by and crush by hazards when the Toyota 8FGCU25 Forklift s/n: 19666 was not removed from service after failing items on a preventative maintenance check. This check failed for worn deck decals, worn forks, and missing horn. Ross Express LLC was previously cited for a violation of this Occupational Safety and Health Standard (1910.178(p)(1)), which was contained in OSHA inspection number 1621768, citation number 1, item number 9-a and was affirmed as a final order on November 7, 2022, with respect to a workplace located at 15 Page Road W, Hermon, Maine.
