29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): a) Diaphragm Molding Press 852: On or about December 11, 2023, and times prior, employees performed maintenance activities such as panel repairs on diaphragm molding press 852. As per 1910.333(b)(2)(iii), employees did not apply a lock and tag on each disconnecting means and per 1910. 333(b)(2)(iv) a verification of deenergization was not performed. The employers lock out tagout procedures under 1910.147 did not address electrical safety hazards and did not incorporate paragraphs (b)(2)(iii)(D) and (b)(2)(iv)(B) of this section.
