29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Building 3, Dock by Overhead Door 47, Americold Logistics LLC, 264 Farrell Road, Syracuse, New York 13209. On or about 8/25/2023, the employer did not provide training for an operator in the safe operation of Raymond Stand Up Powered Industrial Truck, Model 425-C40TT, Serial Number 425-19-6286, resulting in a fractured left foot.
