29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: ' On or about July 28, 2023 at 600 East Clark Street, East Palestine, OH 44113, the employer failed to ensure employees performing trimming operations on the grey piston extruder was protected against being exposed to a moving piston/ram in that employees reach inside the chamber area to trim flashing from the product.
