29 CFR 1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. On or about June 20, 2023, the employer failed to develop and document a control of potentially hazardous energy (lockout) procedure for the Faircrest Steel Plant Vertical Caster West Tundish Car scissors lift type manlift machine attached to the West Tundish Car, when employees and/or outside personnel (contractors) were engaged in service and maintenance activities, such as but not limited to, modifying the West Tundish Car.
