29 CFR 1926.651(c)(2):Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a. Shrock Premier Custom Construction LLC located at 500 N 3rd Street, Coshocton, OH: On or about June 9, 2023, the employer did not ensure a safe means of egress was located in a trench excavation that was more than four feet in depth. Employees were exposed to struck-by hazards while installing storm drainage lines in an unprotected trench that was more than four feet deep. The trench was made up of dirt/mud soil and the end used for egress was sloped which necessitated the use of the employee's hands to egress.
