Powered industrial truck operators did not receive initial training in accordance with paragraph (l) of this section: At D1 Resources, Inc., employees operating forklifts and employees working in the facility were exposed to struck-by hazards. Employees were allowed to operate forklifts in the facility without receiving initial forklift operator training.
