29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On or about May 9, 2023, located at 2A Fiber Line Roll Station 1, the employer did not utilize procedures for the control of hazardous energy when employees were setting up the line by threading through powered rollers by hand, exposing the employees to caught-in/caught-between and ingoing nip points.
