29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. a) Production Line 7 - On or about April 13, 2023, employees were exposed to struck-by and electrical shock hazards in that, the employer failed to ensure that the Pacmac machine (Model # 9500 & Serial # 9500B842) was turned off or shut down while an employee was making adjustments to the products bags.
