1910.28(b)(3)(i) 29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): a) On or about April 13, 2023, and at time prior, the employer failed to ensure that hazardous conditions, including fall hazards, created by holes in the "spider" platform's walking-working surface were corrected or guarded before an employee used the walking-working surface again.
