29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Jobsite located at 800 East Virginia Ave, West Chester, PA: The employer did not ensure that an employee operating PIT Hyster Forklift Model SSCRG360-960P to load sections of a Timberland ASD35 Derrick crane onto a Dorsey flatbed trailer received a combination of formal instruction, practical training, and evaluation of their performance in the workplace, thereby exposing employees to struck-by hazards. This condition was noted on or about March 7, 2023.
