29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): a. On or about February 28, 2023, the employer failed to provide fall protection for employees exposed to fall hazards of approximately 8 feet while mixing and compounding from a walking working surface platform on Littleford 600L mixer located in Plant 1. Employees were required to dump ingredients into a chute opening and the guardrail system had missing components. b. On or about February 28, 2023, the employer failed to provide fall protection for employees exposed to fall hazards of approximately 8 feet while mixing and compounding from a walking working surface platform on Littleford 1200L mixer located in Plant 1. Employees were required to dump ingredients into a chute opening and the guardrail system had missing components. c. On or about February 28, 2023, the employer failed to provide fall protection for employees exposed to fall hazards of approximately 8 feet while retrieving ingredients from a walking working surface platform on Littleford 1200L mixer located in Plant 1. Employees were required to retrieve ingredients from an area with no guardrail system. The storage area did not have a guardrail system. d. On or about February 28, 2023, the employer failed to provide fall protection for employees exposed to fall hazards of approximately 10 feet while mixing and compounding from a walking working surface platform on Littleford 2000L mixer located in Plant 2. Employees were required to dump ingredients into a chute opening and the guardrail system had missing components.
