29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: a) On February 11, 2023, and at times prior, at the 99 Cents Katy facility, the employer did not implement safe work practices such as but not limited to the positive isolation of material and safe line opening procedures before cutting through a 4" pipe. This condition exposed employees to the toxic hazards of ammonia.
