29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: The employer is failing to protect employees from struck-by hazards. This was most recently documented on or about February 1, 2023, at the BCS Stone Showroom facility located at 12900 I Street, Omaha, Nebraska. The employer is using an aftermarket front-end attachments on both a Caterpillar Forklift model EC15: S/N - 3FM00911 and Yale Forklift Model GLC120MGNGBE085: S/N - B818N01678X when raising stone flabs. The forklifts do not have updated plates or markings to identify the change in weight capacity that such attachments cause.
