Located at 157 Kohler St. Pickerington, Ohio: On or about January 31, 2023, the employer did not maintain an effective safety and health program to ensure that hazards were identified and corrected onsite while employees performed residential roofing activities, thereby exposed to falls from approximately twenty feet.
