29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Location: 1 Princeton Road, Burlington, MA On or about 12/22/22, the employer allowed an employee to be lifted by a rough terrain forklift, while working from a wooden pallet on the forks that was being utilized as an elevated working platform. The design and use of the elevated platform was not in compliance with ANSI/ITSDF B56.6-2021, 5.17 requirements for elevating personnel and/or approved by the powered industrial truck manufacturer.
