29 CFR 1926.502(d)(16)(v): Personal fall arrest systems, when stopping a fall, did not have sufficient strength to withstand twice the potential impact energy of an employee free falling a distance of 6 feet (1.8 m) or the free fall distance permitted by the system, whichever is less. At the worksite located at River Miler 185, I-70 Bridge, Rocheport, Missouri: Employees were exposed to fall hazards when erecting bridge pier 13. The personal arrest system did not when stopping a fall have sufficient strength to withstand twice the potential impact energy when Austin Vullo fell from the bridge pier, resulting in him hitting a concrete bridge strut approximately twenty-five feet below and then falling into the river. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.
