29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about August 29, 2022, the employer failed to produce the established procedure so that an employee who was engaged in servicing and maintenance activities could follow the established control of hazardous energy procedure for replacing the Phoenix Power Supply in the Square D Test Cell Switch Gear (53104-1 Square D), located in the first level of the WEEP of the facility located at N7650 County Road LS, Haven, Wisconsin. b. On or about August 30, 2022, the employer failed to produce the established procedure so that an employee who was engaged in servicing and maintenance activities could follow the established control of hazardous energy procedure for replacing the Phoenix Power Supply in the Square D Test Cell Switch Gear (53104-1 Square D), located in the first level of the WEEP of the facility located at N7650 County Road LS, Haven, Wisconsin.
