29 CFR 1910.147(c)(4)(ii)(D): Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. On or about August 8, 2022, the lock out tag out procedure for the BuhlerPrince die cast machine #61 (MN 836CCA, SN DB7004) did not include verification that hazardous energy sources were isolated prior to servicing and maintenance of the machine by employees. Employees were exposed to amputation and crushing hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
