29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; a. On or about July 14, 2022, and at times prior, the energy control procedure for the Grinding Plant 6 - Conveyor Belt #3 did not include specific procedural steps for shutting down and isolating the equipment to ensure that the electrical energy sources with the machine were controlled. As a result, employees were exposed to caught-in, and caught-between hazards associated with moving parts and pinch points on the machinery.
