29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: a) On July 20, 2022: Employees worked inside a 5.8 foot trench box (E0050371) which did not have end caps installed. Trench walls were within 1 foot of the exposed ends of trench box. Note* Abatement certification IS NOT required.
