29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: (a) Main Mill Pad and Facility Wide - On or about July 21, 2022 and on prior dates, the employer failed to evaluate the workplace to determine if spaces were permit-required confined spaces (PRCS) prior to allowing employee-entry into the Mill Assembly to make adjustments, exposing employees to potential machine and atmospheric hazards. This facility has multiple confined spaces, such as, but not limited to the Raw Material Hooper and the Slag Silos, in addition to the Mill Assembly.
