29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On July 5, 2022, in the maintenance shop, employees did not utilize the lock out tag out procedures when trying to identify and diagnose a hydraulic leak in the hopper of the garbage trucks, exposing employees to a struck-by and crushing hazard.
