29 CFR 1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. a. Shop and Laydown Areas: On or about June 3, 2022, and times prior, the employer exposed employees to struck-by hazards, in that powered industrial truck (PIT) training did not include formal training for employees operating the "Clark" model 660L forklift (serial number P232L-0243-9900MP) and "Clark" forklift (serial number P445L-0145-9887KP) who use the forklifts to move heavy machinery.
