29 CFR 1910.134(e)(6)(i)(C):The employer did not obtain a statement that the physician or other licensed health care professional (PLHCP) had provided the employee with a copy of the PLHCP's written recommendation: (a) Location - 19 Varnum Street, Lowell, MA: On or about June 3, 2022, the documentation created and maintained by the employer of employee medical evaluations does not include a statement that the employee has been provided with a copy of the PLHCP's written recommendation. The employer required employees to wear N95 respirators while providing care to suspected and confirmed positive COVID-19 patients.
