29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about June 1, 2022, and at times prior thereto, employees were tasked to enter frac tanks containing used oil-based mud, which were permit required confined spaces, without the use of but not limited to safety goggles, face shields, and hooded Tyvek suits, exposing employees to work related hazards.
