29 CFR 1910.119(e)(3)(i): The Process Hazard Analysis did not address the hazards of the process: On or about April 23, 2022, in the ROSE unit at Valero Meraux refinery located in Meraux, LA, workers were exposed to fire and explosion hazards, when the employer did not identify, evaluate and control the hazardous condition of check valve (CC-013) during an inspection, when it potentially released hydrocarbons from the 8-inch pipe that has no isolation and/or bleeder valve.
