Millbury, MA. 01527. Level 3-1/2 #1 boiler economizer screw. On or about and times prior to March 24, 2022, Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily apparent in area(s) where the economizer screw guards are periodically removed for cleaning and in the event of operation without the guards in place, pose a serious caught in/by hazard.
