29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or before March 21, 2022, employees cleaning sensors, changing out spacer material, and/or repairing or replacing the needle within the robotic head unit of the Lisec VSA-35/25 Fully Automated Applicator were exposed to a struck-by hazard when: (a) Specific hazardous energy control procedures were developed and documented but only listed one of the three energy sources that power the machine and did not include the blocking of the robotic head unit so it would not unexpectedly fall due to gravity. (b) Specific Hazardous energy control procedures were not being utilized in that employees relied on interlocks to shut down the machine and not locking it out.
