29 CFR 1910.147(c)(4)(ii)(B):Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; On or about March 9, 2022, the employer did not have an adequate hazardous energy control procedure for the East Mill mix molder machine that required the pneumatic residual air to be bled off, prior to sanitation by the employee, resulting in the unexpected energization of the shuttle head causing an amputation.
