29 CFR 1910.178(m)(5)(iii):Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered, controls neutralized, and the brakes set to prevent movement: On or about February 11, 2022, at MSC Walbridge Coatings Inc. within the facility located at 30610 E. Broadway Walbridge OH 43465, employees were exposed to struck-by and crushed-by hazards in that the employer did not ensure when operators of an industrial truck are dismounted and within 25 ft. of the truck still in their view, that the load engaging means were fully lowered, controls neutralized, and the brakes set to prevent movement. Employees were exposed to struck-by and crushing hazards when tilting the raised oil totes while standing to the side and in front of them, to refill the oil reservoir tank at Line 25.
