29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) FIMCO, Inc. at 1000 Fimco Lane, North Sioux City, SD 57049: On or about and at times prior to February 4, 2022, the employer did not ensure that the point of operation on the Wallace Bender, Machine number 1100 was guarded. This condition exposed the employee to the point of operation of the machine and resulted in an amputation.
