29 CFR 1910.119(e)(3)(i): The Process Hazard Analysis did not address the hazards of the process: On or about January 15, 2022, in the MDI-1 unit at Rubicon located in Geismar, LA, workers were exposed to thermal burns when the employer did not fully assess and control the release of hot MDI to a safe location after a rupture disc PSE-207 on dechlorination column (AS-4202) had activated.
