29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. On or about December 9, 2021 and at times prior thereto and thereafter, employees used portable ladders for work activities and the employer did not ensure the ladders were inspected before each use exposing the employees to fall hazards.
