1926.502(d)(15)(ii) 29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest systems were not designed, installed and used under the supervision of a qualified person: 29 CFR 1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about December 1, 2021: Employees engaged in framing activities at the Runza construction site located at 8910 Fort St. Omaha, Nebraska, were exposed to fall hazards in that the employees were not using a fall protection system that do not allow the wearer to contact the next lower surface (ground) based upon the configuration of the system. The fall protection system was used in such a way that the anchorage point (unsecured wooden truss) moved allowing the employee to contact the ground and sustain fatal injuries.
