Dock doors 2-27 and 29-33 - On or about October 1, 2021, the employer failed to conduct an assessment of the entire facility to determine all Permit Required Confined Spaces prior to assigning an employee to conduct confined space entry to perform preventative maintenance on the hydraulic dock plates without having implemented a written permit required confined space program. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
