29 CFR 1910.147(c)(4)(ii):Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a. On or about and prior to November 11, 2021, the employer failed to develop specific procedures for the Bale Dumper associated with the Regrind Shredder. The Bale Dumper was newly installed in early 2021 and the employer did not document the hazardous energy control procedures for this equipment. During the performance of maintenance activities covered by this part, authorized employees must control stored mechanical energy subject to gravitational forces and have potential exposure to crushed by injuries associated with the Bale Dumper bin in an elevated position.
