29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) In the 3rd floor boiler section: On or about 10/26/2021, the employer did not ensure that the procedures on which employees were trained were sufficiently understood by an employee performing the work and as such, the procedures did not clearly and specifically outline the scope, purpose and/or techniques to be utilized to control the hazardous energy of thermal heated water stored in condenser #4, prior to an employee starting maintenance/repair work.
