29 CFR OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to contact with anhydrous ammonia from potentially catastrophic releases from the refrigeration system of a facility used for the processing and distribution of meat and seafood which is not effectively inspected, tested, and maintained to ensure its continued serviceability and the pressure relief discharge piping of which does not have a means for draining moisture: (a) Location - 16 Conn Street, Woburn, MA: On or about August 12, 2021, deficiencies in the equipment of the facility's ammonia refrigeration system identified during inspection, testing, and maintenance activities were not corrected in a timely manner nor were alternative means to ensure safe operation employed. (b) Location - 16 Conn Street, Woburn, MA: On or about August 12, 2021, the discharge piping of the pressure relief devices protecting the facility's ammonia refrigeration system does not have a means for draining moisture from the piping. (c) Location - 16 Conn Street, Woburn, MA: On or about August 13, 2021, the ammonia refrigeration system condenser's piping on the roof was not marked to identify it as containing ammonia, the physical state of the ammonia, the relative pressure level of ammonia, the pipe service, and the direction of flow.
