29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: a) On or about August 7, 2021, employees engaged in chimney demolition process were not protected from the hazard of falling approximately 40 feet to the lower level through an unguarded chimney flute opening while working from a tubular welded frame scaffold platform.
