29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Fence Department: On July 30, 2021, maintenance employees authorized to conduct energy isolation procedures did not utilize the documented energy control procedure for the Fence Machine Panel Stacker prior to conducting maintenance activities. The authorized employees failed to notify affected employees of the maintenance activity, did not isolate the energy source(s) and did not apply a lockout device(s) to the energy isolation device.
