Text For Citation: 01 Item/Group: 001A Hazard:
29 CFR 1910.147(c)(4)(ii)(B):The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about July 17, 2021 at the jobsite located at 3105 N. IBP Rd in Holcomb, Kansas 67851 employees engaged in maintenance activities where employees were exposed to caught-in-between hazards associated with hazardous energy from gravity while working on the Stacker Dee. After following the procedural steps for locking and tagging out the Stacker Dee, an employee amputated the tip of their pinky finger on the chain that they were replacing in the Stacker Dee. The Stacker Dee became dislodged from its resting state, falling and catching the employees pinky in the chain. The Lockout/Tagout procedures for the Stacker Dee failed to identify gravity as a hazardous source of energy, leading to the employees injuries.
