(a) On or about July 26, 2021, the employer did not ensure employee(s) unloading trailers and/or box trucks with Crown Electric Pallet Jacks, were trained on the hazards of exceeding pallet/load height to prevent striking the overhead roller doors. This exposed employees to struck-by and crushing hazards.
