29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: For employees working at the Sysco building in Palmetto Florida, procedures to control kinetic energy were not developed when authorized employees were removing and replacing the drive belts on the two cooling tower fans. The fans were spun by a single motor mounted between the fan assemblies. Fan continued to move after the main electrical source had been turned off creating an amputation hazard between the belts and the pulley, on or about July 12, 2021.
